Broad-spectrum antimicrobials are commonly used without indication and contribute to antimicrobial resistance (AMR). We implemented a syndrome-based stewardship intervention in a community hospital that targeted common infectious syndromes and antipseudomonal beta-lactam (APBL) use. Our intervention successfully reduced AMR, C. difficile rates, use of APBLs, and cost.

OBJECTIVE
To determine whether probiotic prophylaxes reduce the odds of Clostridium difficile infection (CDI) in adults and children.
DESIGNIndividual participant data (IPD) meta-analysis of randomized controlled trials (RCTs), adjusting for risk factors.
METHODSWe searched 6 databases and 11 grey literature sources from inception to April 2016. We identified 32 RCTs (n=8,713); among them, 18 RCTs provided IPD (n=6,851 participants) comparing probiotic prophylaxis to placebo or no treatment (standard care). One reviewer prepared the IPD, and 2 reviewers extracted data, rated study quality, and graded evidence quality.
RESULTSProbiotics reduced CDI odds in the unadjusted model (n=6,645; odds ratio [OR] 0.37; 95% confidence interval [CI], 0.25–0.55) and the adjusted model (n=5,074; OR, 0.35; 95% CI, 0.23–0.55). Using 2 or more antibiotics increased the odds of CDI (OR, 2.20; 95% CI, 1.11–4.37), whereas age, sex, hospitalization status, and high-risk antibiotic exposure did not. Adjusted subgroup analyses suggested that, compared to no probiotics, multispecies probiotics were more beneficial than single-species probiotics, as was using probiotics in clinical settings where the CDI risk is ≥5%. Of 18 studies, 14 reported adverse events. In 11 of these 14 studies, the adverse events were retained in the adjusted model. Odds for serious adverse events were similar for both groups in the unadjusted analyses (n=4,990; OR, 1.06; 95% CI, 0.89–1.26) and adjusted analyses (n=4,718; OR, 1.06; 95% CI, 0.89–1.28). Missing outcome data for CDI ranged from 0% to 25.8%. Our analyses were robust to a sensitivity analysis for missingness.
CONCLUSIONSModerate quality (ie, certainty) evidence suggests that probiotic prophylaxis may be a useful and safe CDI prevention strategy, particularly among participants taking 2 or more antibiotics and in hospital settings where the risk of CDI is ≥5%.

The entomopathogenic fungus Metarhizium anisopliae var. acridum is registered as a mycopesticide for acridid control in Africa and Australia. Traditionally, identification of M. anisopliae var. acridum infection in grasshoppers and locusts has relied upon development of fungal growth in infected cadavers. Conventional methods of detection of this entomopathogen in the environment and non-target organisms have been based on culture and bioassay. A PCR-based method for the detection of M. anisopliae var. acridum was developed. Sequence data from the distinct ITS rDNA regions facilitated the design of PCR primers that were used in PCR-based diagnostic assays for the detection of fungal DNA. The amplified sequence was 420 bp in length and specific to M. anisopliae var. acridum. Isolates of M. anisopliae var. anisopliae and M. flavoviride produced no PCR product with these primers. Other fungal entomopathogens, plant pathogens, mycopathogens, and soil saprophytes were also not detected by the pathogen-specific primers. The assay was also effective for the detection of M. anisopliae var. acridum DNA in the presence of soil DNA extracts and in infected grasshoppers.

Summary
In 1796, William Blizard, surgeon to the London Hospital and earnest Anglican, introduced one of his pamphlets with a revealing text. He chose a passage by William Blackstone, the eminent legal commentator, Member of Parliament, and judge.
The General Duties of all Bodies politic, considered in their corporate Capacity, may like those of Natural Bodies, be reduced to the single one, that of acting up to the End or Design, what ever it be, for which they were created by their Founder.
Blizard went on to discuss the topic at hand – the importance of assistant surgeons in hospitals – as an application of this teleological principle. His essay resonated with the analogies he evoked with Blackstone's statement: hospitals were like “all Bodies politic,” like “Natural Bodies,” like God's creation. Inspired founders had shared God's vital spark, infusing their work with an all-encompassing Design. Blizard implicitly played out the rest of the analogy. Just as humans had to figure out God's Design to understand nature, hospital governors needed to grasp the founders' Design to carry out their “General Duties.” Unfortunately, like corrupted churchmen, governors tended to forget “all the great objects they comprehend” and to fail to “appreciate their own true, high relation to the public.” Blizard hastened to remind them that they were responsible “for their conduct to science, to humanity, to all mankind.”
This was rather heavy-handed rhetoric for introducing an argument that the surgeons of the London Hospital needed help with their charitable duties, but Blizard was not one to resist a high moral tone when he held a pen.

In the preface to their first volume of Medical Observations and Inquiries (1757), a “Society of Physicians in London” praised the publications of the Royal Society and the Parisian Academy of Sciences, yet noted that their scope “does not allow them to insert several things … calculated only for the improvement of physick.” Furthermore, the anonymous editor(s) continued, some
important cases … cannot decently be read in mixed company, and there are others, of the merits of which, physicians only can be adequate judges, and which would appear neither entertaining nor instructive to philosophers or mathematicians.
“Mixed company,” a phrase that evokes audiences of men and women, suggests that these physicians had extraordinary delicacy about presenting medical details to nonmedical men, unless prim ladies indeed visited the Royal Society meetings. Note how the physicians claimed to be the only “adequate judges” of their experiences. They could share the “entertaining” or “instructive” case with educated folk, but they were the appropriate interpreters of such matters of fact. Asserting their unique expertise had, of course, always marked medical practitioners, however much they negotiated their authority with patients and peers. So the editor(s) went on to explain the particular grounds for their valuable work:
The persons who formed this Society, were either such as had the care of hospitals, or were otherwise in some degree of repute in their profession; and consequently had frequent opportunities of making observations themselves, and of verifying, in the course of their practice, the discoveries of others. […]

Summary
On 1 May 1740, the Fellows of the Royal Society elected James Hawley, MD, to join them as an FRS. His nomination letter, exhibited at the society from 31 January to 24 April for his potential colleagues to examine, described Dr. Hawley as a
Member [FRCP 1739] of the College of Physicians & Fellow of Oriel College in Oxford. Now residing in Bow Street Covent Garden.
A Gentleman well skilled in Mathematicks, Natural Philosophy, & other parts of Polite Literature, is desirous of becoming a Fellow of the Royal Society, and is recommended as a person who is likely to become a Usefull Member thereof by us.
Signed by four relatively undistinguished members, Hawley's certificate followed a pattern already familiar to the Fellows: this man would fit in. In contrast, those had who nominated John Andrée the previous October had stated that he was a
Doctor of Physick residing in London; who among other ingenious Performances, has publish'd an account of the Tilbury Mineral Waters, and translated from the french [sic] a Treatise on the Venereal Disease, with a Dissertation on the Hydrophobia, and another on Consumptions, written by Dr. Pierre Desault, and being well Qualified, & desirous of the honour of becoming a Member of this Society, is recommended by us.
Andrée's supporters included Sir Hans Sloane, President of the Society, and Cromwell Mortimer, MD, its Secretary. Yet it was unusual to detail publications in nomination letters: perhaps this man did not fit in.

“Sir,” wrote John Wood of Sittingbourne, Kent, on 28 May 1788, to George Neale, surgeon,
I take the liberty in writing to you beging [sic] the favor of a Certificate from you & the other Surgeons of the London Hospital of my being your Dresser in the Above Hospital for twelve Months, my being a Member of the Benevolent Medical Society for the County of Kent I find a Certificate from the Hospital &c may be necessary and that I could wish to have it by the 6 of June. … P.S. the time of my entrance in the London Hospital was October 3 1769.
Nineteen years after walking the London's wards, John Wood found that evidence of that experience might “be necessary” for his practice in Kent. His brief request suggests the little ways that hospital training crept into the making of regular medical men in the eighteenth century. In 1769, Wood chose to pay to watch and assist elite surgeons in their charitable work, spending twelve months engaged in an activity hardly required for a surgeon, or surgeon-apothecary, to start treating patients in Britain. Yet it mattered. In 1788 the “Certificate from the Hospital” was valuable. It marked Wood as a practitioner with an appropriate background, whatever he actually learned under Neale and the other surgeons.
For the emergence of hospital teaching in London, Wood's decision to enter the wards evokes part of the story, the demand side, as it were, of institutional change.

“Charitable knowledge,” I have been told, is a rather peculiar conjunction of words. People are benevolent, generous, lenient, merciful, or philanthropic, giving of their time, compassion, or money; knowledge – well, knowledge, qua knowledge, is not. Yet “charitable knowledge” captures the major arguments in this book. Practitioners in eighteenth-century London hospitals used their medical knowledge and skills for charitable ends. They spent hours voluntarily attending to the sick poor. They tried to help ill men and women to get better, whether or not they were particularly successful with their cures or especially kind to the poor folk under their care. During the eighteenth century, hospital physicians and surgeons made teaching pupils on the wards integral to hospitals' charitable duties. In order to do that, they had to make the daily presence of large numbers of pupils on the wards fit into the meaning of charity for the lay governors whose time and money supported the hospitals. Using hospital patients as teaching objects had to become a good thing to do, a valuable contribution to social betterment for all people, not just a useful experience for a very small number of apprentices and personal assistants.
The modern teaching hospital exists because we – as a culture if not as individuals – in a fundamental sense value the moments when an experienced clinician instructs a medical student using the body and responses of a hospital patient.
